Please complete the forms below that pertain to your child and bring them to the nurse's office:

Asthma Individual Health Plan must be completed for all students with known asthma, must be signed by physician

Asthma - Permission Form to Self- Carry Inhaler

Medication Permission Form must be completed for every medication brought to school

Seizure Individual Health Plan must be completed for all children with a known seizure disorder, must be signed by physician

Diastat Order must be completed by physician

Severe Allergy Action Plan/ Epi-pen Administration Order must be completed for all children with severe allergies requiring Epi-pen, form must be completed by physician

Diabetes Health Care Plan must be completed for all children with diabetes, must be signed by parent and physician
